Text For Citation: 01 Item/Group: 002 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by or crushed by heavy equipment (Bulldozers and Trackhoe machines) when the machine is operating and during repairs: Location: Recycling transfer-station facility located at 151 Anthony St, Brooklyn, N.Y. On or about 04/24/06. c)Horns (audible warning device) were not provided for the two Caterpillar Track- loader (bulldozers) in order to allow the operator to warn of danger or signal to employees in the recycling yard. d)A backup alarm was not provided for the Komatsu 320 Trackhoe machine to warn employees on foot when the machine operates in reverse. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS: Install and use horns and backup alarm on the machines. Horns are provided as a standard safety feature (warning/ signalling device) on the Track Hoe bulldozers by the manufacturer, Caterpillar, and must be maintained operable by employers who use that equipment. Backup alarms are an industry recognized safety signal for bi-directional machines such as the Komatsu 320 Trackhoe machine, as required by ANSI standard Z245.1- 1999 'Safety Requirements for Mobile Wastes and Recyclable Collection, Transportation, and Compaction Equipment' part 7.2.14.1 Warning Signals. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19
