Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a STRUCK-BY HAZARD: a)Well site, Strouse #1A, on or about 12/06/06: The employer failed to ensure that wind guys were flagged with visible markers to prevent contact with heavy equipment. b)Well site, Strouse #1A, on or about 12/06/06: The employer failed to ensure that thimbles were used at the ends of wire ropes securing the wind guy anchoring system to the derrick structure.
