Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to accidents related to hoist failures and falling loads resulting Struck-by/caught- between hazards and related crush and blunt force trauma injuries: A.) Centrifugal Molding Area/-ton Jib Crane #53806/On or About April 25, 2007: The Ingersoll Rand hoist was not equipped with an upper limit over travel device. The Jib Crane and its associated hoist were utilized for work activities that included but were not limited to the construction of centrifugal molds. The Jib Crane's hoist lifted loads that weighed approximately 200 to 500-pounds. Reference: ASME Standard B30.16 Overhead Hoists -Section 16-1.2.14 B.) US Bronze Foundry & Machine, Inc. ---- Hoists: As of August 22, 2007, hoists were being utilized with deficiencies that were identified in the contractor crane/hoist inspection report which was generated in March of 2007. These deficiencies included but were not limited to control cable issues, brake adjustments, pendant labeling, and wire rope issues. Reference: ASME Standard B30.16 Overhead Hoists - Section 16-2.3.3 (a) Abatement Note: Among other methods, feasible and acceptable means to correct the conditions noted above include: 1.Install commercially available upper limit switch as specified and depicted in ANSI/ASME B30.16. 2.Utilize the crane inspection contractor's inspection reports to develop, implement, and follow an abatement plan for the correction of all hoist deficiencies.
