Text For Citation: 01 Item/Group: 002 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to "struck by" hazards from high pressure lines: a) Gray Wolf, Rig 309, Parachute, CO: Located on the mud supply tank, one rigid and one flexible high pressure supply line was not secured to prevent abrupt movement should a line hammer union fail. Among other feasible and acceptable abatement methods to correct this condition would be to attach safety chains or wire ropes on each side of hammer unions.
