Text For Citation: 01 Item/Group: 002 Hazard: CRUSHING

Title 40.1-51.1(A), Code of Virginia: The employer did not furnish employment and a place of employment which is free from recognized hazards that were causing or are likely to cause death or serious physical harm in that employees were exposed to crushing hazards while being lifted in the bucket of a Bobcat 753 Skid Steer Loader to access the second story joists located above the garage attached to a new home under construction. On January 22, 2004, an employee received fatal crushing injuries while standing in the bucket of the Model 753 Bobcat skid steer loader when the bucket was inadvertently raised (instead of lowered) into the second story floor joists. The victim was crushed between the bucket and the floor joists. Feasible and acceptable methods among others to correct this hazard include: Adhere to the Model 753 Bobcat Loader Operation and Maintenance Manual's operator safety warning decal which states "Never use the loader as man lift or elevating device for personnel." (See inside cover and page xiv of the Operation and Maintenance Manual.)
