Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment that was free from recognized hazards that were causing, or likely to cause, death or serious physical harm to employees, in that employees were exposed to struck by hazards: (a) Plant #2 - The employer did not have relief device size justification (calculations) for rupture disks located on reactors D1,D2,R2, and R4. Among other methods, feasible and acceptable abatement methods to correct this hazard are: (1) Perform necessary calculations to determine appropriate size for relief devices and install relief devices that prevent internal pressure from exceeding 10of vessel's MAWP during actual relief. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
