Text For Citation: 01 Item/Group: 002 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries, broken bones, and lacerations as the result of being struck by rolls of paper weighing up to 2010 lbs each: a) The maximum lift capacity (3750 pounds) of the Cascade Paper Roll Clamp, model 45FRCF06B, serial number 90118-01, attached to caterpillar forklift serial number A2EC362196, model 2EC30, Type E, was exceeded when the operator routinely used the paper roll clamp to lift double rolls of paper that weighed in excess of 3750 pounds. Among other methods, feasible and acceptable methods to correct this hazard: Comply with the Cascade Operator's Guide for D, F, & G - Series Fixed Frame Paper Roll Clamps, Manual Number 210135 R-2 and follow the paper roll data plate which lists the maximum lift capacity of the paper roll clamp at 3750 pounds.
