Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the following conditions: a)On or about 01/25/05, and at times thereafter, employees were exposed to caught- between and struck-by hazards because the coil lift car used at the coil slitter line was loaded beyond its rated capacity of 30,000 pounds numerous times. One feasible and acceptable method to abate the condition is to develop, implement and enforce means or procedures to prevent exceeding the design capacity of the slitter and/or other equipment configured to process steel strip from coiled configurations as addressed in clause 9.1.1, ANSI B11.14-1996 Safety Requirements for Construction, Care, and Use of Coil-Slitting Machines, and clause 5.1.3, ANSI B11.18-1997 Safety Requirements for Construction, Care and Use of Machines and Machinery Systems for Processing Strip, Sheet, or Plate from Coiled Configurations, the Cincinnati Slitting Line Specifications and Capacities as outlined in the Cincinnati manual, and by reinstalling the legend or data plate which states the capacity of the coil lift car. *** Certification and Documentation of Abatement Required ***
