Text For Citation: 01 Item/Group: 002 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: a)On or about May 29, 2003, and at times prior, the HEK mast climbing work platform(s) at this location were not installed with the appropriate safety devices, including the mechanical endstop on the mast (red topmast), as well as 'hairpins' to secure the platform. The employer did not have records of the frequent and annual inspections of the platform. One feasible abatement method to correct this hazard is to comply with the erection, inspection, maintenance, operation and record retention requirements of the manufacturer's manuals regarding this platform, and the "Manual of Responsibilities for Dealers, Owners, Users, Operators, Lessors and Lessees of ANSI/SIA A92.9-1993 Mast- Climbing Work Platforms" Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").
