Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to electrical shock in the event of contact with live electrical parts: (a)In the mechanical room - Circuit breaker panel - On or about 05/09/2002, the opening in the panel to which a breaker switch would be intalled was not effectively closed. Among other methods, one feasible and acceptable method would include installation of a blank in the unused opening. (b)At the electrical service meter - Circuit breaker/service meter box - On or about 05/09/2002, the cover to the circuit breaker switches was missing. Among other methods, one feasible and acceptable method would include the intallation of approved covers.
