Text For Citation: 01 Item/Group: 002 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) The hazard of being struck by moving pipe and fittings, in that on or about 02-05-01, a labor crew was doing a pressure test on a 5000 gallon separator tank at a plant under construction. A 1500 psi rated valve was being used and a pressure test of over 2100 pounds was being done on the tank. The valve failed and part of the valve and water under pressure struck the foreman of the crew, killing him. Among other feasible and acceptable methods to correct this hazard or method would be to obtain a valve with a pressure rating at least equal to the test pressure on the tank.
