Text For Citation: 01 Item/Group: 002 Hazard: GUARDING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employee(s) were exposed to the hazard of accidentially entering an unguarded elevator shaft. a)Employees were using the east elevator at 1123 Washington Ave. and it had not been inspected prior to use. The inspection certification had expired June 24, 1999. While the ultimate responsibility for correcting the hazard rests with the employer, given his/her superior knowledge of the workplace, feasible and acceptable methods to correct this hazard, among others, are: (1)The qualified person(s) (Reference ANSI A17.1-1971; Rule 1001.1c) assigned to the job site shall have adequate training and be authorized to recognize and correct elevator deficiencies during routine and/or as needed inspections including, but not limited to the following: (a)Regular elevator inspections shall be conducted annually, and current inspection certification posted. (Reference ANSI A17.1-1971; Rule 1001.1a) (b)Regular elevator inspection shall include checking interlocks for deficiencies to ensure elevator doors are closed during operation. (Reference ANSI A17.1-1971; Rule 609.1) (c)Regular elevator inspection shall include checking handles of lever-type operating devices so that that will return to the stop position when the hand of the operator is removed. (Reference ANSI A17.1-1971; Rule 210.1b) (d)Regular elevator inspection shall include checking appropriate warning signs or symbols are posted such as, but not limited to, "DANGER- ELEVATOR-KEEP DOORS CLOSED." The emergency stop button must be colored red. All control devices shall be clearly and permanently marked as to their function. (Reference ANSI A17.1971, Rule 207.5) Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations(s).
