Text For Citation: 01 Item/Group: 002 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the possability of dropped loads and/or crushing hazards: a) At 616 G St. Greeley, CO. (Platte River By-Products plant site): Employees in the skinning area were exposed to crushing hazards due to the hoists not being given periodic inspections. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: (a) Perform frequent inspections at daily to monthly intervals as per ANSI B30.16-2.1.2 on such items as but not limited to: 1. All controls and operating mechanisms for improper operation; 2. All safety devices for malfunction 3. Electrical cords and pendant for exposure to live conductors; 4. Hooks and other load bearing components of the hoist for deformation, chemical damage, or cracks (b) Perform periodic inspections at monthly to yearly intervals as per ANSI B30-16- 2.1.3, which includes the requirements specified in ANSI B30-16-2.1.2 and the following items but not limited to: 1. Fasteners for loosening; 2. Load blocks, suspension housing, clevices, yokes, shafts, gears, etc for evidence of wear, corrosion, cracks, and distortion; 3. Excessive wear of chain, rope, load sprockets, drums, sheaves, and chain stretch 4. Hooks. Dye penetrants, magnetic particle or other suitabvle crack detecting inspections 5. Electrical apparatus for signs of pitting or any deteriaoration of controller contacts, limit switches, and push button stations 6. Several other areas of examination are listed as well as inspection tables in the ANSI (c) Written, dated, and signed inspection reports and records should be maintained as specified in para. 16-2.1.1(b)(2).
