Text For Citation: 01 Item/Group: 002 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of structural collapse: (a)On or about 24 May 1999, and at times prior thereto, employees were exposed to the hazard of structural collapse of the truss system, since the crane was released from the truss prior to installing all temporary braces. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS TO FOLLOW THE GUIDANCE PROVIDED BY THE TRUSS PLATE INSTITUTE HANDBOOK (HIB-91), PARAGRAPH 4.3.9, OR TRUSS PLATE INSTITUTE POST FRAME SUMMARY SHEET (HIB-98) BY HOLDING THE TRUSS IN PLACE BY THE HOISTING EQUIPMENT UNTIL THE ENDS OF TRUSS ARE SECURELY FASTENED AND TEMPORARY BRACING IS INSTALLED. IN ADDITION, THE COMPANIES TRUSS SETTING PROCEDURE, ADDRESSED IN STEP 3." (b)On or about 24 May 1999, and at times prior thereto, employees were exposed to the potential hazard of a collapse of the truss system, since the trusses were installed with out the use of stringline, plumb bob, level or transit. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS TO FOLLOW THE GUIDANCE PROVIDED BY THE TRUSS PLATE INSTITUTE HANDBOOK (HIB-91), PARAGRAPH 10.1, BY USING A STRINGLINE, PLUMB BOB, LEVEL OR TRANSIT." (c)On or about 24 May 1999, and at times prior thereto, employees were exposed to the hazard of structural collapse due to inadequate diagonal braces. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS TO FOLLOW THE DIAGONAL BRACING GUIDANCE PROVIDED BY THE TRUSS PLATE INSTITUTE POST FRAME SUMMARY SHEET (HIB-98), OR TRUSS PLATE INSTITUTE HANDBOOK (HIB-91), PARAGRAPH 13.1.2.2.OR FOLLOW THE GUIDANCE IN THE COMPANY'S TRUSS SETTING PROCEDURES FOR STEP 2, 3.H, and 4."
