Text For Citation: 01 Item/Group: 002 Hazard: LOCKOUT
SCRR 71-112A: Failed to furnish a place of employment which is free of recognized hazards which may cause death or serious physical harm to his employees and comply with this regulation and other occupational safety and health rules and regulations promulgated under Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as amended, as follows: A. Employer knew or should have known that employees performing welding operations, on or about March 14, 2000, on a piping system which contained, among other substances, turpentine and methanol and which was not locked out, tagged out, or blanked on top of the foul condensate tank were exposed to a fire and/or explosion hazard. A feasible and useful method to correct this hazard, among other methods, is to establish lockout/tagout procedures to control foul condensate which contained methanol and turpentine prior to service and maintenance of condensate recovery system and LVHC piping such as activities which, occurred on or about March 14, 2000.
