North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a)drowning by unexpected life line release. On and before April 16, 1998 at the Oxford Dam, the attachment of the umbilical life line was not equipped with a thimble where polypropylene rope attaches to snap hook. b)drowning by unexpected life line release. On and before April 16, 1998 at the Oxford Dam, the life lines used by employees had knots at the point of attachment to divers. Among other methods, one feasible and acceptable abatement method to correct this hazard is to install thimbles and remove knots from lines or use the guidance provided from the Cordage Institute for attaching to, and sizing of, lines used in critical operations.
