Title 40.1-51.1.(A), Code of Virginia: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: possible mechanical and electrical failure of the Model 45HA, JLG aerial lift used of the job site by an employee at 3444 Western Branch Blvd., Chesapeake, Va., as maintance was not being performed on the machine by the employer.
