Text For Citation: 01 Item/Group: 002 Hazard: LOCKOUT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an amputation hazard. The following condition contributed to the existence of this hazard: a) F/T Ocean Rover: On the factory deck where maintenance of three Badder machines was in progress, and where the work activity exposed the employee(s) to a potential hazard of amputation due to accidental re-energization while working on the unguarded chains and sprockets, the machines had not been fully and completely isolated from their electrical energy isolating device, i.e., circuit breaker and/or disconnect switch to prevent the inadvertent transmission or release of the electrical energy. Only the machine's on/off switch, main emergency stop switch and one additional safety limit switch was utilized to render each Badder machine inoperative. Note: One feasible means of abatement, among others, would be to disconnect each machine from its Energy isolating device at the breaker and/or disconnect switch, then lockout/tagout the device./tagout
