Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of electrical shock, burns and or electrocution while using a defectively wired receptacle as a power source for electrical tools/appliances: (a)M/V Independence, engineers tool room: An electrical outlet, mounted on an overhead beam and on the bulkhead, was improperly wired and indicated reverse polarity when tested. Note: One feasible method, among others, is to reconnect the wiring to the appropriate terminals and test the outlet for correct reading before use.
