Text For Citation: 01 Item/Group: 002 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crush and fall hazards: a) Manoa Shopping Center, Interior, West Side - Employee(s) was exposed to crush and fall hazards in that he was operating an elevated scissor lift in an area that contained uncovered/unguarded floor hole(s). On 12/11/1997 an employee who was operating the MX15/19 Upright scissor lift while being elevated approximately 13 feet above the floor, entered an uncovered/unguarded floor hole causing the unit (and the employee in it) to tip over. The employee sustained serious injury. Observed 12/11/97. ABATEMENT NOTE: Among other methods, one feasible and acceptable means of abatement is to have all floor holes appropriately covered or to maintain a safe distance from obstacles, debris, drop-offs, HOLES, depressions, ramps, and other hazards to ensure safe elevated travel as per ANSI A92.6-1990 section 8.10.15.
