Military Road Bridge Renovation Project, Span Two - Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognizable hazards that were causing or likely to cause death or serious physical harm to employees, in that: a.only nine of the ten bolts used to attach the northern most bracket to Pier one were fully embedded per the manufacturer's specifications. b.the contractor did not reasonably account for the questionable structural integrity of the concrete in the area on which the bracket was installed. Among other methods, one feasible and acceptable means of abatement would be to install all ten required bolts to the specified depth and to perform an adequate survey (other than visual observations) to determine the structural integrity of the concrete where the northern most bracket was installed on Pier one.
