29 CFR 1910.23(a)(3)(i): Hinged floor opening cover of standard strength and construction equipped with standard railingsor permanently attached thereto so as to leave only one exposed side. When the opening is not in use, the cover shall be closed or the exposed sideshall be guarded at both top and intermediate positions by removable standard railings: Employees performing housekeeping and maintenance tasks were exposed to fall hazards up to 28 feet. The floor openings for fixed industrial ladders did not have hatches for the following locations: a) Opening on the upper floor of the pellet mill, next to the cyclone dust collector to the ground floor. b) Ladder from distributor platform in the grain elevator to the bin deck.
