29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. (a) At the USP UNICOR Laundry area, the authorized employee failed to lock the electrical power panel while maintenance employees were working on the clothes washer. Employees were exposed to mechanical and electrical hazards while maintaining and repairing the clothes washer.
