Longmore Rd Water Line Excavation: On or about 23 January 2026, the employer failed to ensure that the Volvo track hoe (04022430) being used to excavate a water line was equipped with a reverse signal alarm distinguishable from the surrounding noise level or have a spotter employee signal it was safe to do so.
