Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) The hazard of death or serious physical harm from exposure to steam; or, components of the hydroclave that accidently came off the unit on or about 02/08/89. Among other methods to correct this, one feasible and acceptable method is to develop, institute, and maintain an accident prevention program comprised of the following items; 1) Training in both routine and emergency procedures; 2) A review of hazards involved in the process and operation of new pressure vessels. 3) Scheduling periodic inspections to correct and identify unsafe conditions and work practices with documentation of these inspections. 4) Provide operating instructions for hydroclaves for each employee involved with the operation whether it be during set-up operations or production runs.
