29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about October 3rd, 2025, employees engaged in demolition operations located 1701 West Pleasant Hill Road, Lincoln, Nebraska 68523, were exposed to struck-by hazards in that the employer failed to provide adequate training to employees on how to safely remove an existing brick wall. As a result, an employee was fatally struct by a brick wall that partially collapsed.
