29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A.) Warren General Hospital, On or about 8/10/25and times prior thereto: Employees did not lockout and relieve stored hydraulic energy prior to performing preventative maintenance work on equipment including but not limited to pump 18. An employees fingers contacted a spinning propellor resulting in a fingertip avulsion and possible amputation. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
