29 CFR 1910.334(c)(3): Test instruments and equipment and their accessories were not rated for the circuits and equipment to which they were connected and were not designed for the environment they were used. a.) Ellwood City Forge Company; 800 Commercial Avenue Ellwood City, PA 16117, on or about 08/24/2025, and times prior thereto: Employees were working inside the General Electric 4160 volts T2/T3 switchgear in the Millwright Shop Room. Employee was using a 600 volt Fluke clamp meter inside the energized 4160 volt switchgear. The Fluke Clamp Meter was only rated for 600 Volts. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
