29 CFR 1910.119(e)(6): The employer did not update and revalidate by a team meeting the requirements in paragraph (e)(4) of this section, to assure that the process hazard analysis is consistent with the current process, at least every five (5) years after the completion of the initial process hazard analysis: a) At the Clairton Plant, on or about August 11, 2025 - The employer did not update and revalidate the 1998 Underfire Gas System Operability and PHA Report at least every five years. The most recent updated and revalidated process hazard analysis of the Underfire Gas System was conducted in November of 2018. Failure to update and revalidate the initial PHA of the Underfire Gas System at least every five years exposed employees working in and around the process to an explosion hazard when cryogenically-processed coke oven gas was released from 13 Pad Valve.
