29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146: On or about August 1, 2025, and at times prior, on a site located at 4529 Hypoluxo Road, Lake Worth, Florida, the employer did not develop and implement a written permit-required confined space entry program for employees exposed to hazardous chemical vapors such as, but not limited to Benzene, Toluene and Merican 9505 Unsaturated Isophthalic Resin when entering an underground fuel tank to perform a repair.
