29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury. On or about July 31, 2025, at the worksite located at 305 E. Spring St., Saint Marys, Ohio: The employer did not instruct employees in the recognition and avoidance of unsafe conditions related to temporary shoring and supporting a multi-story building. Employees were tasked to remove a section of the south wall, install temporary shoring and headers, and excavate the foundation in preparation for footers to transfer the building loads to shoring systems. Employees were not instructed in the means and methods for building and installing a temporary shoring system, in the recognition and avoidance of unsafe conditions related to demolition work, in monitoring methods to ensure the stability of the building during demolition and construction work, or to identify and detect hazards resulting from weakened walls or floors. The condition exposed employees to struck-by and crushing hazards. No additional abatement information is needed for this item.
