29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about July 1st, 2025, employees engaged in roofing operations located at 5104 North 210th Ave, Elkhorn, Nebraska 68022, were exposed to fall hazards in that employees were using an extension ladder for access/egress to and from the roof and the ladder did not extend at least 3 feet above the upper landing area.
