29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: Residential Sewer Connection Project, 7 Hamilton Court, Norwich, CT: On June 13, 2025, the employer did not ensure the employee was provided a stairway, ladder, or ramps for a safe means of egress to be protected from cave-in, struck-by, and atmospheric hazards while working between 4 feet 10 inches to 5 feet 4 inches deep trench performing sewer pipe installation/laborer activities, resulting in fatal injuries
