29 CFR 1910.178(l)(2)(i)(A): The employer did not ensure trainees operated a powered industrial truck only under the direct supervision of persons who have the knowledge, training, and experience to train operators and evaluate their competence: On or about May 15, 2025, a temporary worker in training was not directly supervised at all times during the operation of a powered industrial truck in the warehouse located at 1013 Oak Street, Elwood, Kansas, thus was exposed to crushing hazards.
