29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy source: On or about May 7, 2025, the employer did not ensure the electrical energy isolating device (cord and plug) was operated in a manner to isolate the electrical power of a Sheffield C-530 Hot & Cold Automatic Strip Cutting machine, while a CKM (cut, kit, mark) operator performed service and maintenance (unjamming) operations.
