29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access: The employer is failing to protect employees from fall hazards. This was most recently documented on or about April 8, 2025 when roofers were observed installing shingles and working at height. Roofers were exposed to a fall distance of 11 feet eave to ground in that the Aluminum ladder used to access the roof was not extended at least 3 feet above the eave and was not secured to the structure.
