29 CFR 1910.22(d)(2): The employer did not ensure that hazardous conditions on walking-working surfaces are corrected or repaired before an employee uses the walking-working surface again. The employer did not ensure that if the correction or repair cannot be made immediately, the hazard is guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired. At 1461 Kin Ark Ct, Saint Louis, MO, 63132, On March 31, 2025 and at times prior thereto, employees were exposed to struck by and crushing hazards in the loading yard area. The loading yard area was discovered to have multiple large water-filled holes, which made the walking-working surface uneven. Employees, vehicles, and powered industrial truck traffic with loads were traversing through the area. The loading area was not guarded to prevent employees from using the walking-working surface until the holes could be repaired.
