29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): (a) On and prior to March 10, 2025, employees were exposed to an unexpected release of energy while cleaning the saw box of the Schurman Machine Incorporated Edger Saw Model No. 6TA60 Serial No. 2512 in that the specific procedure did not include information on how to isolate the hydraulic energy. (b) On and prior to March 10, 2025, employees were exposed to an unexpected release of energy while cleaning the saw box of the Schurman Machine Incorporated Edger Saw Model No. 6TA60 Serial No. 2512 in that the specific procedure did not clearly identify the type of energy isolating device to be used on the control power of the operator's console. (c) On and prior to March 10, 2025, employees were exposed to an unexpected release of energy while cleaning the saw box of the Schurman Machine Incorporated Edger Saw Model No. 6TA60 Serial No. 2512 in that the specific procedure did not include locking out the "buckets" for the scan and infeed chains.
