29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): (a)Sentry LLC at 1928 Calvin Drive, Watertown, SD 57201: On or about and at times prior to February 5, 2025, the employer did not ensure employees were protected from stuck-by hazards in that, not all forklift operators were trained, evaluated and certified by the employer prior to operating the Clark Forklift, model NPX17, s/n NPX345 0867 9701KY on site. This condition exposed employees to struck-by hazards associated with improper forklift usage.
