29 CFR 1910.29(b)(2): The employer did not ensure that guardrail systems met the requirement that midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members were installed between the walking-working surface and the top edge of the guardrail system when there is not a wall or parapet that is at least 21 inches (53 cm) high: 194 Cleveland St. Greenfield, MA: On or about January 24, 2025, and at times prior, employees were exposed to fall hazards of approximately 24-feet above a lower level when performing work activities on a walking surface with a guardrail system installed, but no mid-rail or equivalent member installed between the walking surface and the top rail.
