29 CFR 1910.146(d)(6):The employer did not provide at least one attendant outside the permit space into which entry was authorized for the duration of entry operations: (a) 402 Tatamy Rd., Nazareth, PA - Dryer/Kiln: On or about January 23, 2025, employees engaged in welding, torching and grinding inside the dryer/kiln were exposed to entrapment and hazardous atmospheric conditions as a result of the employer's failure to provide at least one attendant outside the permit space into which entry was authorized for the duration of the entry operations.
