On or about January 22, 2025, the employer failed to ensure that forklifts that were found in need of repair were removed from service. The LP Toyota, Model 42-5FG25, Serial Number 405PG25-15941 was allowed to be operated while having a leaking hydraulic cylinder and inoperable horn, thereby exposing affected employee(s) to struck-by hazards.
