29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: 29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a)Rapid Packaging Company, Inc. at 1310 West 7th Avenue, Brookings, SD 57006: On or about and at times prior to January 7, 2025, the employer did not ensure procedures were utilized to control potentially hazardous energy prior to employees conducting maintenance on the Hycorr 66x115" Prographics 2 color Flexographic Printing Rotary Die Cutter. This condition exposed employees to laceration and amputation hazards. (b)Rapid Packaging Company, Inc. at 1310 West 7th Avenue, Brookings, SD 57006: On or about and at times prior to January 7, 2025, the employer did not ensure procedures were utilized to control potentially hazardous energy prior to employees conducting maintenance on the General Gluer Serial: L5095. This condition exposed employees to laceration and amputation hazards. (c)Rapid Packaging Company, Inc. at 1310 West 7th Avenue, Brookings, SD 57006: On or about and at times prior to January 7, 2025, the employer did not ensure procedures were utilized to control potentially hazardous energy prior to employees conducting maintenance on the Bandsaw 9874. This condition exposed employees to laceration and amputation hazards. (d)Rapid Packaging Company, Inc. at 1310 West 7th Avenue, Brookings, SD 57006: On or about and at times prior to January 7, 2025, the employer did not ensure procedures were utilized to control potentially hazardous energy prior to employees conducting maintenance on the Slitter Model: RJE 120" 4 Bar Slitter. This condition exposed employees to laceration and amputation hazards.
