Text For Citation: 01 Item/Group: 002 Hazard:
29 CFR 1910.147(d)(3):All energy isolating devices that were needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy source: (a) On or about October 30, 2024, at the facility located at 3935 Westgate Road in Grand Island, Nebraska: The employer failed to isolate all energy sources on the Caterpillar 259B3 Compact Track Loader during a maintenance activity. A maintenance employee elevated the 380kg control arm assembly to access and remove the right and left control arm bushings. The raised assembly was not isolated from forward movement that occurred when the maintenance employee removed the control arm bushings. The employee suffered a fatal injury as a result of this uncontrolled energy release.
