29 CFR 1926.702(j)(1): Employees were permitted to perform maintenance or repair activity on equipment (such as compressors, mixers, screens, or pumps used for concrete and masonry construction activities) where the accidental start-up of the equipment was not eliminated by use of lock out/tag out procedures: a) On December 3, 2024, at 3338 Congen Road, Labelle, Florida, the employer did not ensure lock out/tag out (LOTO) procedures for the Rapid-Mix 400 CW mixers of Mobile Batch Plant Number 4 were implemented and followed to prevent the accidental equipment start-up while cleaning the mixers, thus exposing employees to caught-between and crushing hazards.
