29 CFR 1915.152(b)(4):The employer failed to document an occupational hazard assessment which should have included: Occupations involved in the survey, the date in which the survey took place and the name of the person who performed the hazard assessment: a) Atlantic Oceanic: On or about 11/7/24, the Employer did not provide a verification document showing that a PPE hazard assessment had been conducted. Abatement certification and documentation are required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
