29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D): a) On or about November 7, 2024, at the East side of the back middle area of the warehouse roof: employees were exposed to an 18'-6" fall hazard while working on the roof to repair a leak, in that the employer did not ensure that each employee was protected from falling through a skylight.
