On or about 10/29/2024, the employer did not provide employees with the information contained in Appendix D of the Respiratory Protection Standard ("Information for Employees Using Respirators When Not Required Under the Standard"), when the employer permitted employees to use a 3M Model 8511, N95 Filtering Face-Piece Respirator.
