29 CFR 1910.269(m)(3)(i): The employee that the employer designates pursuant to paragraph (m)(2) of this section as being in charge of the clearance did not make a request of the system operator to deenergize the particular section of line or equipment, as the designated employee in charge (as this term is used in paragraph (m)(3) of this section) and responsible for the clearance: a) Location-320 West Main St., Sayreville, NJ -JCP&L Substation, On or about 5/24/2024, a TRC Environmental Corporation contracted employee sustained electrical burns due to an arc flash while performing air quality testing within a JCP&L electrical power sub-station. A JCP&L supervisor and qualified employee designated to escort the TRC Environmental Corporation employee into the substation did not verify the building was de-energized prior to the start of work.
