29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows: (i) as part of a complete personal fall arrest system which maintains a safety factor of at least two; and (ii) under the supervision of a qualified person. a) On or about October 3, 2024, at 2797 Shaffer Avenue, Cincinnati, Ohio, the employer failed to ensure that the anchor points used for attaching personal fall arrest equipment were capable of supporting 5,000 pounds (22.2 kN) as required in that, anchor points were installed by nailing ropes directly into the roof decking. As a result, employees working on the roof, approximately 13 feet above an asphalt driveway and hard soil, were exposed to a fall hazard without adequate fall protection.
